Born on December 1st\, 1968 in DeSoto\, TX\, Jan a died from complications due to breast cancer on April 25\, 2015 in Broom field\, CO. Jana is survived by her mother Janice\, husband Ken and her ch ildren Carter and Angelica. In her 47 years of life\, Jana was the embodi ment of optimism\, lifting the spirit of everyone she came into contact wi th. Jana traveled the world\, provided a safe and loving home to other peo ple’s kids\, and remained unfathomably positive in the face of cancer.\n\n This scholarship is restricted to individuals who reside in the service ar ea of the Colorado Chapter of the National Bleeding Disorders Foundation. The Jana Marie Lambert Memorial Scholarship Application must be received a t the NBDF Colorado office by June 30th\, 2026–no exceptions\, electronic submissions only. Applications completed by anyone other than the applican t will not be accepted. Incomplete applications will not be considered. Se lected recipients will be contacted after July 15\, 2026. Payments will be made directly to the institution specified in the application and under n o circumstances will funds be directly provided to individuals. \n\nPlease be sure to attach all supporting documentation in the area provided below .
